Last meeting: 16 September 2008 - Delivering joined-up, seamless health and social care across a multiplicity of providers: how to gain efficiencies and avoid duplication (members only) About the group There is currently no organised way of sharing information between appropriate Healthcare and Social Care agencies. This process of recording and accessing patient or client information separately means that for example, Social Workers have little direct information on a clients involvement with their GP and vice versa. Therapy staff, home care workers and mental health professionals hold their own separate records. Hospital appointments, admissions and procedures are, similarly, separately recorded and stored.

The Health and Social Care group aims to get a clearer understanding of what is taking place across health and social care sectors now, what the future of integration will look like and how Information Technology can facilitate this integration.
